This session explores real-world case studies highlighting adolescent and adult CF diagnoses, CRMS conversion, and powerful insights from a patient’s personal journey. Presenters will showcase two distinct program approaches:
1. An individualized stepwise strategy to support patients and families through new diagnoses
2. A patient advisory board-driven initiative focused on quality improvement to enhance education and outreach following newborn screening.
Attendees will gain practical tools to improve patient engagement and care across the lifespan.

Learning Objectives:

Describe at least two case studies that illustrate the challenges and considerations involved in diagnosing cystic fibrosis (CF) in adolescents and adults, including CRMS conversion and patient perspectives.
Identify the key components of a stepwise, individualized approach to supporting patients through a new CF diagnosis.
Evaluate the impact of a quality improvement initiative and patient advisory board project aimed at improving education and outreach for families following a newborn CF diagnosis.
