W37--Managing CF, One Transition at A Time

This workshop will focus on important considerations of caring for people with CF (PWF) and their families during various transitions across the lifespan. Themes discussed will include the transitions to becoming a caregiver of a child with CF, how to navigate school while living with CF, transitioning from pediatric to adult CF care, critical needs for PWCF as they age, and the importance of advance care planning in the transition towards more serious health concerns. This session will incorporate the presentation of novel data and clinical initiatives to improve the understanding of the unique needs during transitional periods in the lives of PWCF and their families.

Learning Objectives:

  • Describe clinical data obtained from PWCF and their families related to priorities and areas for concern that should be addressed by members of CF care teams during critical transition periods.
  • Identify suggestions for improving CF care based on the data and clinical initiatives presented.
  • Discuss how findings presented impact PWCF and CF care team members across both pediatric and adult care settings.
