S15--From Crisis to Care: Tackling the Lung Transplant Journey

This session highlights psychosocial considerations for multidisciplinary team members supporting patients with CF (PwCF) as they consider bilateral orthotopic lung transplant (BOLT), prepare for formal candidacy application, and navigate the often-murky world that is the "status-post BOLT" clinical landscape. The symposium will include general guidance for providers seeking to provide comprehensive, patient-centered care for patients with a special focus on a) best practices for integrated and palliative care teams, b) health disparities, c) and patient perspectives.

Learning Objectives:

  • Consider ways to bolster integrated care teams in order to minimize the impact of symptoms and treatment side effects on the quality of life and mental health of Persons with Cystic Fibrosis, both pre- and post-BOLT.
  • Examine the ways in which health disparities present barriers to equitable bilateral orthotopic lung transplant (BOLT) care and identify ways teams can seek to minimize the impact of these barriers on overall patient care and prognosis.
  • Present a patient perspective on the lived experience of undergoing BOLT to optimize our psychosocial support of these patients.
