SPC03--Role of Airway Clearance Techniques including Exercise as an ACT in an Era of Change

This session will focus on the use of evidence-based ACT from birth through adolescence to adulthood in the era of CFTR modulators. This will include the use of exercise as ACT, PEP and Oscillating PEP Techniques, IPV, HFCWO, and other mechanical devices available. Appropriate positioning, family dynamics, adherence issues, and managing the asymptomatic patient together with those with advancing lung disease will be included. This Special Class will also focus on those who qualify for CFTR Modulators. There will be practical sessions with breakout groups to discuss issues and management of the different ages, life-stages (including pregnancy), pathophysiology and treatment strategies.

Learning Objectives:

  • Discuss the uses of the different types of evidence-based Modern ACT available, including Active Cycle of Breathing Technique; Autogenic Drainage; Positive Expiratory Pressure (PEP); Oscillating PEP and external oscillating devices; physical exercise as ACT – for those on highly effective CFTR modulators and those who do not qualify
  • Describe situations when ACT is contra-indicated or to be used with caution and/or modified e.g., hemoptysis, pneumothorax, gastroesophageal reflux, segmental collapse/consolidation or mucus retention/plugging
  • Explain how CFTR modulators may influence different types of ACT (including exercise as ACT and the need for huffing) and their dosage when pwCF are stable and with acute exacerbations with mild, moderate, and severe lung disease
